As a client of Therapy with Jeanine, you have the right to request a copy of your mental health care and medical records.
Send a written request via email directly to the practice.
Please include your full legal name, date of birth, and specific records being requested.
Your request will be processed in compliance with state and federal laws governing confidentiality and access, including Texas Health & Safety Code § 611.0045. A reasonable, cost-based fee may apply for records preparation and delivery.
